How to cut chiropractic new-patient no-shows

An empty new-patient slot is not a flaky town. It is a booking window and a confirmation habit. Both are under your control.

Show rate on new patients is exams that happened divided by exams that were scheduled. Eighty percent is healthy. Under 80 percent is leaking enough to feel it in the week. Under 60 percent is an emergency. You do not buy more Facebook leads until that number moves. More leads at a 50 percent show rate buy more holes in the book.

The three fixes

Book the exam inside about two days. Same day, next day, or the day after. A new patient who waits a week has time to feel slightly better and to decide the drive is not worth it. If the next 48 hours are full, you have a capacity problem. Add a block or a doctor before you add an ad budget. A calendar that offers "the 19th" as the first opening is manufacturing no-shows.

A person confirms. The front desk, not a no-reply address. They confirm the time, where to park, what to bring, and that the visit is an exam with a recommendation at the end, not a mystery. Day before and morning of. Reminders from software can exist. They are not the system.

Rebook the miss the same day. The slot passed. Someone calls or texts before they leave the building for the night and offers two times inside the same short window. A missed new patient is warmer than the next ad lead. Treating them like a dead file is how clinics stay "busy" and miss plan starts. After the same-day attempt, they move to the longer follow-up list, not into oblivion.

The money

Thirty new-patient exams booked. At 50 percent show, 15 happen. At 80 percent, 24 happen. If half of shown exams start a plan worth $1,000 in the first stretch of care, that gap is several plans a month from people you already acquired. The front desk time to confirm is not in the same league as the ad spend you were about to add. Tie this to the cost-per-plan sheet in Facebook ads for chiropractors so the owner sees why the marketing report looked fine and the deposits did not.

Count it honestly. A patient who arrives, fills out nothing, and leaves is not a show. A patient who reschedules before the day, with a new time, is not a no-show. A patient who ghosts is. If one doctor's new patients show and another's do not, look at how those slots were set and confirmed before you look at clinical skill. Clinical skill shows up on the people who sat down. The plan conversation, once they are in the chair, is the care plan. Do not coach the doctor on plans while half the book is empty. Fix the empty book.

What makes no-shows worse

A special that felt free and a visit that feels like a surprise sales call. People skip appointments they are dreading. Tell them, in the confirmation, that they will get a recommendation and a choice. Match that to the new-patient offer so the visit is what you promised. Price range included before they drive. Surprises create cancellations the morning of, which is a no-show with manners.

Make the front desk prove it for seven days before you buy another lead. Each morning the schedule shows yesterday's new-patient misses in red until a new time is on the book or the patient declined in writing. Each exam today shows a confirmation timestamp from a person, not from software. The doctor does not start the day by asking where the 8 a.m. went. The desk already knows, and the rebook is in motion. If you have two locations, do not average them into one comforting number. A clinic at 85 percent can hide a clinic at 40 percent. Post both. The weak one gets the owner on confirmations for a week so the standard is visible, then the desk keeps it. Capacity is the only acceptable reason to book past two days, and "the doctor likes a light Thursday" is not capacity.
